The goal of home health care is to treat an illness or injury and help you get better, regain independence, and become as self-sufficient as possible, all while allowing you to recover at home.
Home health care providers can provides services such as care for pressure sores or a surgical wound, patient and caregiver education, intravenous or nutrition therapy, injections and monitoring serious illness and unstable health status.
How does someone qualify for home health services?
A referral is needed for home health care, and you need to be established with a primary care physician. Once your care provider refers you to home health services, a member of our team will come to your home to talk about your needs and ask you some questions about your health to determine the best plan for treatment. You also must be “homebound,” meaning it is either medically contraindicated for you to leave the home, or it’s a severely taxing and tiring effort for you to get out and about.
